LBPH APPLICATION

The LBPH application is downloadable/printable at www.loc.gov/nls. Printing the
application is still required because our program still requires an original, actual
certifying signature.

WE have print copies on file, but this might be useful, particularly if someone with
internet access calls rather than comes in.
